General Information
Patients are seen by appointment to allow for the amount of time necessary for a thorough examination and consultation. Your consultation includes a thorough review of your medical history a physical examination and might also include additional testing such as blood, urine and semen analysis as well as ultrasound examination. Our office hours include early evening and weekend appointments for your convenience.
A medical history questionnaire is completed by each patient prior to consultation. It is important for you to complete this form as honestly and completely as possible. If you have any questions related to this form please contact our office prior to your visit. If you have had any tests or studies previously, please bring the report of the results with you.
